浙大王立教授和俞豪杰教授團(tuán)隊(duì) ACS AMI:用于糖尿病治療的靜電作用增強(qiáng)的葡萄糖響應(yīng)胰島素遞送微針貼

  糖尿病是一種由于患者胰島素分泌功能缺失或受損而導(dǎo)致的以高血糖為主要特征的代謝性疾病。根據(jù)國(guó)際糖尿病聯(lián)合會(huì)的調(diào)查顯示,全世界糖尿病患者的人數(shù)已超過(guò)5.37億。目前對(duì)于I型和重癥II型糖尿病的治療需要每日注射2~4胰島素以維持正常血糖水平,給患者帶來(lái)注射疼痛和生活不便,同時(shí)由于無(wú)法按需要實(shí)時(shí)給予合適劑量的胰島素,存在過(guò)量給藥低血糖風(fēng)險(xiǎn)。葡萄糖響應(yīng)微針貼可根據(jù)血糖水平自適應(yīng)調(diào)節(jié)胰島素釋放速率,在糖尿病治療領(lǐng)域受到廣泛關(guān)注。


  浙江大學(xué)王立教授和俞豪杰教授團(tuán)隊(duì)長(zhǎng)期從事葡萄糖響應(yīng)胰島素遞送水凝膠及微針貼方面的研究。20237月他們團(tuán)隊(duì)通過(guò)親疏水作用和靜電作用設(shè)計(jì)了一種葡萄糖響應(yīng)負(fù)載胰島素納米微粒并制備了胰島素遞送微針貼,實(shí)現(xiàn)了糖尿病大鼠模擬一日三餐進(jìn)食條件下的血糖控制(ACS Appl. Mater. Interfaces 2023, 15, 31330-31343)。近日,他們報(bào)道了一種對(duì)葡萄糖濃度波動(dòng)響應(yīng)更靈敏、制備更簡(jiǎn)便、安全性更好的苯硼酸基葡萄糖響應(yīng)凝膠微針貼ACS Appl. Mater. Interfaces 2024, 16, 4449-4461。他們基于4-羧基-3-氟苯硼酸改性ε-聚賴氨酸(FPBA-PL)與聚乙烯醇(PVA)構(gòu)建了動(dòng)態(tài)苯硼酸酯鍵交聯(lián)水凝膠網(wǎng)絡(luò),利用質(zhì)子化后帶正電的氨基對(duì)胰島素的靜電吸引和動(dòng)態(tài)硼酸酯鍵解離對(duì)胰島素釋放的促進(jìn)作用,可以通過(guò)調(diào)節(jié)水凝膠中氨基和動(dòng)態(tài)苯硼酸酯鍵的含量?jī)?yōu)化胰島素釋放對(duì)葡萄糖響應(yīng)性(Figure 1),在糖尿病大鼠模擬一日三餐進(jìn)食的條件下,單片微針貼能夠?qū)崟r(shí)維持其血糖在正常水平24小時(shí)。

  體外胰島素釋放實(shí)驗(yàn)結(jié)果表明,通過(guò)優(yōu)化FPBA-PL/PVA比例以及FPBA-PLFPBA的接枝率顯著提高了胰島素釋放的葡萄糖響應(yīng)靈敏性(Figure 2)。

  基于糖尿病大鼠模型考察了所得苯硼酸基葡萄糖響應(yīng)凝膠微針貼GR-MNP的血糖控制性能(Figure 3)。結(jié)果表明,微針貼GR-MNP具有優(yōu)異的透皮胰島素遞送和血糖控制能力。與直接注射胰島素(INS組)或使用非葡萄糖響應(yīng)微針貼的nGR-MNP組相比,GR-MNP組具有更加穩(wěn)定和長(zhǎng)效的血糖控制性能。在糖尿病大鼠模擬一日三餐進(jìn)食的條件下,1GR-MNP微針貼就可以維持血糖濃度在正常范圍內(nèi)約24小時(shí),血糖水平基本與對(duì)照組的健康大鼠相同。
